Amsler Grid
Have the patient wear their glasses. Cover one eye. Hold the chart 18 inches from the patient, and
have the patient look directly at the small black dot in the center of the grid. Record any areas of
breaks in the lines or distortions of the lines. Repeat with the other eye. Normal is no breaks or
distortions in the lines. Any new findings should be reported at once.
